Your website inquiry
When you use the OPC contact form, we receive the name, email address, phone number and message you choose to provide. An email address is required so we can respond. Please leave out sensitive health or payment information.
Your submission is stored in OPC’s HighLevel/LeadConnector account with its submission time and source information. An email notification containing your inquiry is sent to Nick and Hangry Local Marketing, which helps OPC manage the website and inquiries. Copies are also stored in their email accounts. We use this information to respond to your request and manage your inquiry.
Website and form services
HighLevel/LeadConnector provides the website and embedded inquiry form. The site also loads hosted images, videos and supporting scripts to display the pages and operate the form. These services receive technical information needed to process your browser’s requests.
The form uses Cloudflare Turnstile to help detect automated submissions. Turnstile processes signals about your browser and device to provide this security check. See Cloudflare’s Turnstile Privacy Addendum.
Google Maps
The Contact page embeds Google Maps to show the OPC facility. Loading or interacting with this map connects your browser to Google and may share browser/device information with Google. Get Directions opens Google Maps. Google’s privacy practices apply to its services.
PocketSuite and external links
The Recovery Membership link opens PocketSuite, where membership, payment and applicable forms are handled separately. Recovery sessions must still be scheduled through Noel. Review the terms and privacy information provided in PocketSuite before using its services. Other external links, including Instagram, open services with their own privacy practices.
